Patient Forms For Eye Exams
For patients who booked an eye exam recently, we kindly ask that patients fill out their forms ahead of time to maximize appointment time with the optometrist. Each form takes about 3-5 minutes to complete. If you are unable to fill them out at home, we recommend to arrive 10 minutes earlier to your appointment to complete them.
New Patients
Never had an eye exam with our doctor? Please fill out 2 forms below. The first form will ask you to document any existing medical conditions and prior eye surgeries. The second will review our policies and ask you questions about discomfort related to your vision.
Existing patients
Seen our doctor once before? Please fill out only 1 form below. We're checking in on your overall health and documenting any new symptoms.
